- Wrong dimensions — Canada uses portrait format (50×70 mm, approximately 2×2¾ inches), not the square US-style 2×2 inch photo. Using the wrong format is the most common mistake.
- Head height outside 31–36 mm — your head from chin to crown must measure 31–36 mm within the frame (44–51% of photo height). Too close or too far from camera causes rejection.
